Maybe try checking to make sure there are no DATA conflicts like duplicates for example.

The only data problem is a possible missing child.

When you request the ordinances, try requesting only the Endowment - uncheck the SP. See if it will reserve both of them for you.
There is a possible duplicate on the mother - you may want to see if you can resolve that duplicate first.
