Missing Temple Ordinances

Vicarious temple ordinances were done for Taija Chiba 1879-1948 GNQG-S43 (B 29 May 1981, E 1 June 1981, and SP 4 June 1981) at the Oakland Temple. They do not appear under "ordinances" on his person page. How do I get these dates entered into Family Tree? Do I need to contact the Oakland Temple? I have not saved the temple name cards from so long ago. There do not appear to be any duplicates that need to be merged.
