Permission to do ordinances

I would like to reserve the temple work for my deceased friend. I have written permission from his closest living relative.

Yes I have completed all the steps. Should I attach the written permission here? I have everything else done.

I apologize, I probably should not have checked that my question was answered. I think we're close. Do I submit the Name, ID Numbers, written permissions and names of those giving permissions here?
