permission to do ordinances

Hello. I spoke with a wonderful sister a couple of days ago about doing ordinances for my friend who passed away a year ago, who has no family members who are members of the Church. I have printed the request for permission and am sending it to her son (who has already given his permission on Facebook Messenger). I am including a stamped self-addressed envelope. I need to know what the address should be on that envelope. I figure it would be best if he send it to you, or am I not correct in that assumption? My name is Linda Hyde. Many thanks!
