Hazel has an Oregon delayed birth certificate, from 1958. https://www.ancestry.com/sharing/29990051?h=34a8ee The California Death Index shows that she died there, not in Oregon, in 1990. https://www.familysearch.org/ark:/61903/1:1:VPXG-T92
And I see you just found the California Death index and attached it, while I was also searching.
