In a case of when they put down the wife went back to her maiden name, in this case no surname was put by the wife's name, do you put the maiden name down for the wife? When they did put down the married surname, do you put the maiden name before the married name?

Yes to both. Image 1 will be Agnes Mary MacDonald. For your second question, if her last name was listed as Madigan under maiden name, with the same remarks, it would be MacDonald Madigan.

• Do not assume the surname of an individual based on the surname of someone else, such as a parent or a spouse, unless directed otherwise in the project instructions.
• Type the maiden name before the current or former surnames. A maiden name was often indicated by the words “née” or “born as.” A maiden name may also have been written in parentheses.
