@Nancy Wright_3, thank you so much for you question! When the surname field says "Field Unavailable," which causes you no to be able to put the surname in the field, please look above on the page and see "Report a Problem". When you click on that, you will see options to put a check in. The second option is "The surname field is unavailable". Please check that and submit it. Thank you so much!!!
