What is the difference between Family Ordinace Request & Family Name Cards?

When requesting a temple ordinance I see 2 options.

What would be the difference?

Is FOR just a list of family name cards?

Short Answer:

If one CANNOT print "Temple" Cards oneself; THEN, one can print a "Family Ordinance Request" (FOR) instead.

One can take, the FOR, to a "Temple"; where, "Temple" Workers, print the "Temple" Cards, for one.

But ...

That Said ...

ONLY, if the "Temple", is operating, under 'Phase 4' "Conditions" - ie. 'No', COVID-19 Restrictions ...
