Same problem as others have stated because any state comes up. These have no birth locations that it should be looking at.
Enumerator's Signature also has a comma, as if the format of that field was supposed to last name, first name

Yes, there are comma's in the name. Please correct and take the comma out and put the first name followed by the last name. If there are fields, then put the first name in the correct field, etc. Thanks again.
