Ordinances "printed" by the temple years ago never completed

I have a similar issue with a few names on my reserved list except with baptisms/confirmations. Only one of the two ordinances was "printed" by the temple years ago and never was completed. My guess is that those papers are lost in the temple somewhere now. The IDs are: LV2R-J16, LV2R-V34, LV2R-VZH, LVTG-XVV, L2KS-543.
