Ordinances Done Years ago Not Showing Completed

When entering baptism and initiatory ordinance dates just completed into my Family TreeMaker software on my computer, I discovered we had done the temple work for Margaretha Falkenstein LT43-HVS previously but FamilySearch didn't show the previous dates so I have started duplicating them. I think probably merges and deletions have caused the problem but there is no way for me to type in the ordinance dates so I am attaching the cards showing the ordinances done previously. Can someone in the Temple Department please put the dates into the Family Tree at FamilySearch? The year for baptism & confirmation is difficult to read on the card - it is 19 May 2004. Thank YOU!
