I do not have a paper copy to prove it, hoping you can find it. He has been merged and unmerged because his name is common.
His ordinances should be:
B&C 30 May 2006 Boise
E 10 Oct 2006 Boise
FS temple shows some ordinances done with newer dates and that he needs his Endowment done, if these dates are corrected there would not be a need to duplicate his temple work.
