Could you please tell me if the above England Herefordshire Transcripts should have a surname on burials

that have a child or wife with first name only if it does give father or husband's surname? It might be different

than the normal rule of omitting them. I want to be doing it correctly. Thank you!

In the Field Helps for Deceased's Surname it says

If a surname was not recorded for a deceased child and only 1 surname was recorded for both parents, type the parents' surname in this field. If 2 different surnames were recorded for the parents, type the father's surname in this field. This is an exception to the General Indexing Guidelines.

I believe this would apply to a wife as well.

I didn't get to see your batch, but, that instruction would only apply to the child since the instruction only says deceased child. If the wife is deceased, you would not index the husband's surname as her surname as per our normal indexing guidelines. But, I think that the moderators are checking on these field helps. Stay tuned...
