How did I get the back side of death certificate that refers to notes over on back of certificate.

I would think that that the note that says to "Read Explanations on back carefully" have to do with the completing all fields that you see on the front. In other words, they are instructions for the person who is filling out the death certificate, not necessarily for the recipient - you! All the information you need is showing clearly on the front.

Diana, thank you for your response. Not sure if you can direct me in this request. I have searched for my grandfather Agustin Jaramillo "Agustin Gutierrez" burial site with no luck. Can you help me or direct me in this search. I'm trying to search for his family too. This has been a long process over the years. He was my mother's biological father. I appreciate your direction and help if possible.

@Cynthia Rivas Start with what you know and work backwards from there. From the death certificate you posted here, it names Augustin's wife as Guadalupe Bachachi. Based on their names and residence, this is a possible record for him and his wife in the 1930 US Census: https://www.familysearch.org/ark:/61903/1:1:XH1B-ZBT.
